Turning a Failed Chatbot Launch into a Governed Reinvestment Case

A national retail chain’s customer service chatbot was pulled offline six weeks after launch following a string of inaccurate responses — and leadership was ready to write off conversational AI as too risky to revisit.

Client challenge

The chatbot had been rushed to launch without a clear escalation path for uncertain answers, without content review of its underlying knowledge base, and without a monitoring process to catch drift. Its failure had also damaged internal trust in AI projects broadly, making it harder to get funding approved for any future initiative.

Our solution

We ran a structured post-mortem to separate what actually went wrong (ungoverned content sourcing and no human escalation path) from what leadership had concluded (that the technology itself was unreliable). From there, we built a governance model for conversational AI specifically: mandatory content review before any answer source went live, confidence thresholds that routed uncertain queries to a human agent, and a weekly accuracy audit.

We packaged this into a business case that separated the relaunch into three phases, each with its own success gate, so leadership could greenlight phase one without committing to the full rollout up front.

Results

The relaunched chatbot shipped with a governed content pipeline and human escalation for roughly 18% of conversations flagged as low-confidence. Six months post-relaunch it was handling 61% of customer service inquiries without escalation, at a customer satisfaction score 4 points higher than the original launch, and leadership approved funding for two additional conversational AI use cases on the strength of the governed relaunch.
